Instructions:
Membership in the Ohio Cremation & Memorial Society (OCMS) costs only $25.00 for a lifetime membership! You can register just yourself or, your can register both you and your spouse for $25.00 each.

We have a Short Form and a Long Form. The long form contains detailed bio and preference information. The short form is simply a contact form with some very basic information. If you choose the short form, we will contact you for the remaining information.

We will contact you for payment, payment can be made by check or by Credit Card (Visa, MasterCard, Discover).
